Substituting players help!

1) What happens if I active every substitution for suppose at 60' , which one will take place as I know there are 3 sub limit ?

2) What happens if an injury takes place after 3 subs completed or a substituted player gets injured who will take his place?

2.1) For example I use 4-4-2 formation and I subbed both of my SC , and a substituted player gets injured who will take his place?

3) If 2 players are subbed already at a fixed time for example 30' and then a player gets injured at 43'  will the substitution at 60' take place or the injury will count as a substitute and sub will not happen?

According to my information, the answer to 2nd point is your team will play with ten players.

3rd)60' substitution won't happen then
